You must complete these requirements to get all the 32BJ Maternity Program benefits:

✓ Choose a hospital and an OBGYN or midwife who participate in the 32BJ Maternity Program. Please visit www.32BJmaternity.org to see the full list of hospitals and their providers.

✓ Enroll in WebTPA’s Healthy Beginnings program by calling them at 866-266-8122.

✓ Call 32BJ Health Services at 866-230-3225 by Week 32 of your pregnancy to tell us which hospital and OBGYN/midwife you have selected and to let us know that you have enrolled in Healthy Beginnings.

After you meet the program requirements, you will be eligible for these benefits:

• Gift Package: As soon as you complete the requirements above, we’ll call you to choose your gift and confirm your mailing address. You have until 8 weeks after your delivery to choose your gift. But please don’t wait! Please visit www.32BJmaternity.org to see the available gift packages.
• $0 Copay: Eligible moms who have enrolled in the 32BJ Maternity Program and deliver at a 32BJ Maternity Program hospital will have a $0 copay for the admission for their delivery.
